There's been a lot of buzz about the new meaningful use standards announcements this past week. My quick take on it is that if Stage 2 criteria were a phone, it'd be less like the iPhone 5 people were expecting, and more like the upgraded iPhone 4S they wound up with. In other words, tweaked, not revolutionary.

In my opinion, the user-experience-enhancing "Siri" of the piece (if I'm to carry the phone analogy too far -- work with me here!) is the upcoming requirement that more than half of a given organization's patients be provided timely online access to their health information, and that ten percent must actually access that information.

I mean, we've been able to use the Internet to access our bank records for some time now. Why shouldn't we be able to have the same kind of access to our medical records? I really think there should also be some sort of an appeals process when things are mis-charted, or misdiagnosis has occurred, but I won't be holding my breath until that shows up in the meaningful use guidelines.

  • Advance Directives

    In all the talks on "patient-centered healthcare" and "patient empowerment," it seems that everyone has completely overlooked the fact that there is no higher level of patient-centered care than actually asking a patient what treatment he/she wants, and then honoring those wishes to the extent possible. Ignoring the wishes of patients cannot continue if we are going to save our healthcare system. Therefore, I think that it is critical that the meaningful use guidelines follow the strong recommendation of the HIT Policy Committee that advance directives be made a core requirement of Meaningful Use Stage 2 for all EPs, EHs and CAHs, and the requirement should be that >50% of patients >49 yrs old seen for an annual physical during the reporting period have the result of an advance directive discussion recorded in the EHR, and the advance directive itself if one exists. In addition, the criteria should be that CMS-certified EHRs should be required to enable online documentation and patient/health proxy signature of advance directives. The EHR should be required to directly access the advance directive, not simply "indicate where it might be located." This HIT Policy Committee recommendation is being COMPLETELY IGNORED in the proposed Stage 2 Meaningful Use rule.

  • Patient Engagement can't be overemphasized

    I think more emphasis should be put on patient engagement. It's really the only hope to improve outcomes (and slay the healthcare cost beast) for 3/4 of U.S. healthcare spend -- chronic conditions.

    It's a failure of imagination of the vendor community if we can't come up with dozens of ways for providers to surpass the 10% engagement threshold. Any # of companies could help them. It will be a catalyst to finally bring in the most important member of the care team into the care process -- the patient. It can be done.
