Outbound blocking -- irrelevant idea
Here's the reality. If you (the user) can do something, a program on your computer can do exactly that same thing. So, if you can turn off your firewall, a worm can do the same thing. If you can click "Yes" on a dialog, a worm can do the same thing. If Windows Firewall included outbound filtering, the first thing a worm would do is turn it off.

The only reason that ZoneAlarm and others seem to work today is that they all have different ways to be turned off, so worm writers haven't written in support for them. If Windows Firewall had outbound filtering, the pure prevalence of the product would cause a worm to be written to turn it off within 10 seconds of release.

Rather than waste resources on what is sure to be a doomed effort, Microsoft has chosen to focus on efforts to make sure that the worm never gets on your machine in the first place.
