If the problems are honest, I think this is a result of expansive programming. The more complicated a program and the more complicated its micro-functions, the more likely the chances to run into failure increase, especially when testing time remains either finite or proportional (to development time) or both.
If the problems are rigged, you know where to throw the stones.
