You missed one critical point

You should also add "enforcement," to the list
of role based access.

Data loss products such as Vontu(Symantec) can
do this for you, and there are a whole slew of
others that can as well(Sophos, McAfee, etc).
Identify the data on your network, show
permissions, classify the data, audit access,
notify upon access, and even proactively block
access to data.

All of your points are valid, but if there is
no active enforcement to block access, then
being reactive is being too late.
