Zmud: "It will mean less pay for physicians and that will lead to a shortage just like it has in Canada and the UK".
Wrong. The shortage of doctors in Canada is not due to low pay. It is due to two factors: (1) The liberal government of some time ago drastically reduced incentives to become doctor. Less influx in medical schools = less output of doctors later (=now); (2) The medical professional associations furiously defend their turf: they have a vested interest in keeping the number of doctors low - and therefore salaries high. Yes, salaries are high.
Blogger: "So those who opt out are cheating everyone else".
Wrong. They make a choice not to consume medical services (except for emergency care); alternatively they pay for it themselves.
