Dutch tax office IT failure

A software problem at the Dutch tax office recently forced 400,000 businesses to refile their payroll details, according to Dutch News: The chaos has been caused because software supposed to make it easier for firms to file payroll details electronically does not work properly.While the UK has enjoyed more than its share of major IT failures (for example, click here and here), I haven't seen IT problems in the Netherlands government.

The chaos has been caused because software supposed to make it easier for firms to file payroll details electronically does not work properly.

While the UK has enjoyed more than its share of major IT failures (for example, click here and here), I haven't seen IT problems in the Netherlands government.

This failure makes me wonder whether the software development and deployment teams conducted testing before the full-scale rollout. I tried to research this, but couldn't find additional information. If you have more details, please leave a comment.
